Nécrose de la Carotide Commune Secondaire à un Corps Étranger Migrant

Abstract

RÉSUMÉ
Nous rapportons notre premier cas de nécrose de la carotide commune droite secondaire à un corps étranger œsophagien migrant, chez un patient de 51 ans, admis dans un contexte d’urgence pour détresse respiratoire. Le délai de sa symptomatologie était de 2 mois marquée par l’ingestion de corps étranger. Quelques jours plutard, apparait une tuméfaction antéro-latérocervicale, une dyspnée, une dysphagie et une fièvre. L’examen clinique et l’imagerie nous a permis une orientation diagnostique et la confirmation a été faite en per-cervicotomie mettant en évidence une nécrose de la carotide commune droite et un gros hématome compressif. Après l’échec de plusieurs tentatives de réparation de la carotide commune et une ligature de celle-ci fût faite suivi de la fermeture du plan cutané et le pansement. Les soins post-opératoires ont été planifiés en collaboration pluridisciplinaire en milieu de soins intensif. A J1, les suites opératoires ont été marquées par le décès du patient.
ABSTRACT
We report our first migrant foreign body complicated by right common carotid necrosis in a 51-year-old patient admitted to an emergency setting for respiratory distress. The delay in symptomatology was 2 months marked by ingestion of foreign bodies, antero-laterocervical swelling, dyspnea, dysphagia and a fever. The clinical examination and the imaging allowed us a diagnostic orientation and the confirmation was made by exploratory per-cervicotomy a necrosis of the right common carotid and a large compressive hematoma. After the failure of several attempts to repair the common carotid artery and a ligature of it was made followed by the closure of the skin and the dressing. Post-operative care was planned in multidisciplinary collaboration in an intensive care setting. On D1, the operative suites were marked by the death of the patient.
