Abstract

RÉSUMÉ
Introduction. Le but de cette étude était de décrire les aspects épidémiologiques, cliniques, évolutifs et thérapeutiques des accidents vasculaires cérébraux de l’enfant à Pointe-Noire. Patients et méthodes. Il s’est agi d’une étude descriptive et prospective pendant 35 mois à l’hôpital général de Loandjili. Tous les patients de moins de 18 ans hospitalisés pour un AVC ont été inclus. Résultats. Nous avons colligé 16 dossiers d’enfants qui présentaient un accident vasculaire cérébral ischémique et un cas d’accident cérébral hémorragique. L’âge moyen des enfants était de 11 ans avec des extrêmes de 7 à 15 ans. Le début de la symptomatologie était brutal. L’hémiplégie et les troubles de langage étaient les signes cliniques les plus fréquents (53% des cas). Le territoire de l’artère cérébrale moyenne était concerné dans 81,3% des cas et l’hématome cérébral était de localisation profonde. Les principales étiologies étaient : les cardiopathies emboligènes, les causes hématologiques et les causes infectieuses. Deux patients sont décédés à la phase aigüe et l’évolution a été favorable dans la majorité des cas. Conclusion. L’AVC de l’enfant est rare, il n’existe pas de différence sur le plan clinique entre celui de l’enfant et de l’adulte. Les étiologies dépendent du contexte épidémiologique.


ABSTRACT
Introduction. The aim of our study was to describe the clinical fatures, management and outcome of childhood stroke in Pointe-Noire. Patients and methods. It was a cross sectional descriptive and prospective study over 35 months at Loandjili General Hospital. All patients under 18 hospitalized for stroke were included. Results. We collected 16 files of children who presented with ischemic stroke and 1 with hemorrhagic stroke. The average age was 11 years with extremes of 7 to 15 years. The onset of symptoms was sudden. Hemiplegia and speech disorders were the most common clinical signs (53% of cases). The middle cerebral artery was mostly affected (81.3%) and the cerebral hematoma was of deep localization. The main etiologies were: Embologenic heart disease, hematologic causes and infectious causes. Two patients died in the acute phase and the outcome was favorable for the rest of patients. Conclusion. Childhood stroke is rare. There is no significant difference of clinical presentation between childhood stroke and adult stroke. The aetiologies depend on the epidemiological context.
