Abstract

RÉSUMÉ
Introduction. Le but de cette étude était  d’étudier la fertilité et le devenir des grossesses post-myomectomie par laparotomie dans deux hôpitaux de première catégorie dans la ville de Douala, Cameroun. Méthodologie. Il s’agissait d’une étude descriptive rétrospective incluant les dossiers des patientes ayant subi une myomectomie par laparotomie du 1er janvier 2012 au 31 décembre 2020 à l’Hôpital Général et à l’Hôpital Gynéco-Obstétrique et Pédiatrique de Douala. Les données épidémiologiques et cliniques, le nombre et le devenir des grossesses  étaient recueillis. Les résultats ont été analysés à l’aide du logiciel SPSS version 25 et Microsoft Excel 2016. Résultats. La moyenne d’âge des patientes était de 37,02 ±4,90 ans avec des extrêmes de 25 et 50 ans. La proportion de grossesse après myomectomie était de 16,2%. Les grossesses après procréation médicalement assistée représentaient 21,2%. Le délai moyen de conception était de 18,75±10,72 mois. Nous avons recensé 72,7% d’accouchements à terme, 15,2% de fausses couches, et 9,1% d’accouchement prématuré. La césarienne a été réalisée chez 70,4% des parturientes. Nous avons observé un cas de rupture utérine, compliqué d’hémorragie du postpartum et de mortalité périnatale. Conclusion. La survenue d’une grossesse après myomectomie par laparotomie est observée chez 16,2% des patientes. Le délai moyen de conception est de 18,75±10,72 mois. Le pronostic obstétrical des grossesses est globalement favorable. En effet, près des ¾ d’entre elles s’achèvent par un accouchement à terme d’un fœtus viable par césarienne. Il importe de garder à l’esprit le risque de rupture utérine au pronostic péjoratif.
 
ABSTRACT
Introduction. The aim of our study was to assess fertility and pregnancy outcome after myomectomy by laparotomy in our area.  Materials and methods. We conducted a descriptive retrospective study at the Douala General Hospital and at the Douala Gyneco-Obstetric and Pediatric Hospital. It included women who underwent myomectomy by laparotomy from January the 1st, 2012 to December the 31st, 2020. Epidemio-clinical aspects, pregnancy rate and outcome were registered into a file. Those data were analysed using SPSS version 25 and  Microsoft Excel 2016 softwares. Results. Overall, 204 patients were included.  After the operation 33 patients became pregnant, which constitutes 16.2%, within which, 7 (21.2%), after medically assisted procreation. The average delay of conception was 18,75±10,72 months. Pregnancy ended with birth at term in 24 patients (72.7%), spontaneous abortion occurred in 5 patients (15.2%), and pre-term delivery in 3 cases (9.1%). Delivery were done through caesarean section in 70,4% of the patients. One case of uterine rupture with perinatal death were noted. Conclusion. The pregnancy rate after myomectomy in our area is 16.2%. The average delay of conception is 18,75±10,72 months. Most of the pregnancies end with birth at term of life babies through caesarean section.
