Abstract

RÉSUMÉ
Nous rapportons un cas de placenta accréta révélé par une hémorragie massive consécutive à une aspiration manuelle intra-utérine pour une grossesse arrêtée de 14 semaines d’aménorrhées chez une patiente drépanocytaire SC de 33 ans ayant un antécédent de trois césariennes et d’un curetage endo-utérin. Le diagnostic était posé au décours d’une laparotomie exploratrice et confirmé par l’examen anatomopathologique. Le traitement chirurgical à consisté en une hystérectomie d’homostase. La patiente mourut une heure plus tard dans un contexte de choc septique et de défaillance multiviscérale. Ce diagnostic rare n’est pas à méconnaître au premier trimestre du fait de l’augmentation constante du taux des césariennes.


ABSTRACT
We report a case of placenta accreta revealed by a massive hemorrhage consecutive to an intrauterine manual aspiration for a non progress pregnancy at 14 weeks of amenorrhea in a 33-year-old sickle cell SC patient with a history of three cesareans and an uterus curetage. The diagnosis was made after exploring laparotomy and confirmed by the anatomopathological examination. Surgical treatment consisted of a homostasis hysterectomy. The patient died an hour later in a context of septic shock and multivisceral failure. This rare diagnosis is not to be disregarded in the first trimester due to the constant increase in the rate of cesareans.
