Abstract
RÉSUMÉ
L’éléphantiasis péno-scrotal est caractérisé par une infiltration lymphooedémateuse du scrotum pouvant s’étendre au pénis et au pubis. En Afrique, les causes étaient initialement dominées par l’étiologie filarienne notamment dans les régions endémiques, mais aujourd’hui les causes urétrales et idiopathiques partagent l’essentielle des étiologies. Nous rapportons à travers ces observations, deux nouveaux cas d’éléphantiasis idiopathiques péno-scrotaux chez deux patients jeunes de 35 et 49 ans. Le diagnostic était essentiellement clinique. On notait une augmentation du volume du scrotum, une perte de son élasticité, devenant dure, infiltrée. Cette infiltration s’étendant au pénis, mais aussi au pubis chez le premier patient. La peau était néanmoins intègre dans les deux cas. Notre premier patient présentait une infiltration scrotale et de la région pubienne, le pénis n’était pas atteint. Notre second patient avait un lymphœdème important à l’origine d’un enfouissement pénien. Nous avons réalisé chez nos patients une plastie péno-scrotale selon la technique de Butreau-Roussel modifiée selon les spécificités de nos patients. Les suites opératoires étaient simples, avec un recul de 1 an.
ABSTRACT
Penoscrotal elephantiasis is characterised by lymphoedematous infiltration of the scrotum, which may extend to the penis and pubis. In Africa, the causes were initially dominated by filarial aetiology, particularly in endemic regions, but today urethral and idiopathic causes share the majority of aetiologies. We report two new cases of idiopathic penoscrotal elephantiasis in two young patients aged 35 and 49 years. The diagnosis was essentially clinical. The scrotum increased in volume and lost its elasticity, becoming hard and infiltrated. This infiltration extended to the penis, but also to the pubis in the first patient. However, the skin was intact in both cases. Our first patient presented with infiltration of the scrotum and pubic region, but the penis was not affected. Our second patient had significant lymphoedema causing penile burying. We performed a penoscrotal plasty using the Butreau-Roussel technique, modified to suit the specific needs of our patients. The post-operative course was straightforward, with a follow-up of 1 year.
