Pancréatite Aiguë Associée à une Perforation Gastrique : À Propos d’un Cas: Acute Pancreatitis and Gastric Perforation: A Case Report

RÉSUMÉ
La pancréatite aiguë est une inflammation sévère du pancréas, bien que rare, représente une urgence médico-chirurgicale grave, particulièrement lorsqu'elle est associée à une péritonite causée par une perforation gastrique. Ce type d'association, extrêmement rare, présente un taux élevé de morbidité et de mortalité malgré les avancées en réanimation. Nous rapportons un cas rare de pancréatite aiguë associée à une perforation gastrique chez un patient de 36 ans admis aux urgences pour douleur abdominale aiguë, hyperthermie et vomissements. Le diagnostic de pancréatite aiguë a été confirmé par des niveaux élevés de lipase et d'amylase, et un scanner a révélé un pneumopéritoine et une nécrose pancréatique. Une laparotomie a révélé une perforation gastrique, nécessitant une réparation chirurgicale et un drainage. Le patient a reçu une antibiothérapie et un soutien hémodynamique en réanimation, avec une évolution post-opératoire favorable. Ce cas souligne la complexité de l'association entre pancréatite aiguë et perforation gastrique, nécessitant une intervention multidisciplinaire rapide pour une prise en charge optimale.

ABSTRACT
Acute pancreatitis, a severe inflammation of the pancreas, although rare, represents a serious medical-surgical emergency, particularly when associated with peritonitis caused by gastric perforation. This type of association, extremely rare, presents a high morbidity and mortality rate despite advances in resuscitation. We report a rare case of acute pancreatitis associated with gastric perforation in a 36-year-old patient admitted to the emergency department for acute abdominal pain, hyperthermia and vomiting. The diagnosis of acute pancreatitis was confirmed by elevated lipase and amylase levels, and a CT scan revealed pneumoperitoneum and pancreatic necrosis. A laparotomy revealed a gastric perforation, requiring surgical repair and drainage. The patient received antibiotic therapy and hemodynamic support in the ICU, with a favorable post-operative outcome. This case highlights the complexity of the association between acute pancreatitis and gastric perforation, requiring rapid multidisciplinary intervention for optimal management.
