Abstract


RÉSUMÉ
Objectif. Décrire les caractéristiques cliniques et étiologiques de l’endophtalmie. Patients et Méthodes. Étude transversale rétrospective et descriptive réalisée sur une période de 10 ans allant de Janvier 2008 à Décembre 2017 à l’HIARAY. Était inclus tout patient suivi pour endophtalmie cliniquement avérée et ayant bénéficié d’un examen ophtalmologique complet avec ou sans examens complémentaires. Les variables étudiées étaient épidémiologiques (âge, sexe), cliniques (motif et délais de consultation, antécédents, acuité visuelle, signes cliniques), para cliniques (imagerie) et étiologiques (mode de contamination, biologie). Résultats. Trente patients sur 18255 (0,16%) ont été retenus. L’âge moyen était de 47,86 ± 21,66 ans avec une prédominance masculine (sex ratio= 1.5). L’endophtalmie était unilatérale dans 100% de cas. 70% de patients avaient consulté entre 3 et 30 jours. Les signes fonctionnels étaient la douleur dans 80% de cas et la baisse d’acuité visuelle dans 100 %. L’examen clinique a trouvé une hyperhémie conjonctivale dans 93,33% de cas, un abcès de cornée dans 50% de cas et un hypopion dans 66,66% de cas. Une hyalite antérieure à 3 croix était observée chez 6,66% des patients. Les principales étiologies étaient les traumatismes (50%) dont 30% d’origine végétale. La cécité survenait à 93,40% et était totale dans 36,70% des cas. Conclusion. Les endophtalmies sont des pathologies rares dans notre milieu avec une prévalence hospitalière de 0.16 %. Elle est principalement d’étiologie traumatique et aux conséquences graves car elles sont à l’origine d’une cécité monoculaire chez neuf patients sur 10.
ABSTRACT
Objective. To describe the clinical features and etiologies of endophthalmitis. Patients and Methods. We did a cross sectional retrospective and descriptive study covering a period of 10 years from January 2008 to December 2017 at HIARAY. We included all records of patients with clinically proven endophthalmitis, who underwent a comprehensive ophthalmological examination with or without paramedical tests. The variables of interest were clinical features (age, sex, reason and time for consultation, history, visual acuity, and clinical signs), imaging findings and etiologies (contamination mode, biology. Results. Thirty patients out of 18,255 (0.16%) were retained. The average age was 47.86 ± 21.66 years and the sex ratio M/F was 1.5. The endophthalmitis was unilateral in 100% of cases. The consultation delay ranged from 3 to 30 days for 70% of patients. The main symptoms were pain (80%) and reduced visual acuity (100%). Clinical examination revealed conjunctival hyperemia in 93.33% of cases, corneal abscess in 50% and hypopion in 66.66%. Anterior hyalitis (3 crosses° was observed in 6.66%. The leading etiology was trauma found in 50% of cases out of which 30% were of vegetal origin. Visual impairment was recorded in 93.40% of cases while blindness was total in 36.70%. Conclusion. Endophthalmitis is rare, often unilateral and trauma being is the main etiology in our environment. The visual prognosis is poor.
