Abstract

RÉSUMÉ
Introduction. La fibrillation auriculaire est le trouble du rythme le plus fréquent. Elle représente un véritable problème en milieu cardiologique. L’objectif de notre étude était de décrire la sévérité des symptômes selon la classification EHRA, les aspects thérapeutiques et évolutifs des cas de FA hospitalisés. Matériels et Méthodes. Nous avons réalisé une étude transversale rétrospective descriptive de janvier 2018 à décembre 2019 dans le service de Cardiologie du CHU de Kati. Ont été inclus dans l’étude les patients de tout âge et des deux sexes ayant présenté une FA sur des critères cliniques et électrocardiographique, admis dans le service pendant la période d’étude. Les variables étudiées étaient : les caractéristiques sociodémographiques, la classification de la FA, la classification EHRA, le risque thrombo embolique, le risque de saignement, le traitement et les complications observées. Résultats. Au total 52 patients ont été inclus. La tranche d’âge > 60 ans (57,70%) a été prédominante. Le sexe féminin a représenté 69,23 % des cas soit un sex ratio de 0,44. La FA permanente (69,23%) était la plus retrouvée. La classe EHRA III (40,38%) a été la plus représentée. Le score de CHA2DS2-VASc était ≥ 2 chez la majorité (80,77%). Le score de HAS-BLED était ≤ 2 chez la plus grande partie (75,01%) des cas. Les principaux traitements étaient : l’héparine de bas poids moléculaire (76,92%), les AVK (94,22%), le Beta bloquant. (90,38%). La complication la plus fréquente était : l’insuffisance cardiaque (53,84%). Le taux de létalité était de 5,76%. Conclusion : La fibrillation auriculaire est fréquente dans notre milieu, les mesures préventives et une prise en charge efficace doivent être mise en place afin d’éviter ces complications qui sont potentiellement graves et invalidantes.


ABSTRACT
Introduction. Atrial fibrillation (AF) is the most common permanent rhythm disorder. It represents a real problem in the cardiological environment. The objective of our study was to determine the severity of symptoms according to the EHRA classification, the therapeutic and evolutionary aspects of AF cases hospitalized. Materials and Methods. We carried out a descriptive retrospective study from January 2018 to December 2019 in the Cardiology department of the University Hospital of Kati. Included in the study were patients of all ages and both sexes with clinical and EKG-related AF who were admitted to the department during the study period. The variables studied were: socio-demographic characteristics, AF classification, EHRA classification, thromboembolic risk, the risk of bleeding, treatment and the complications observed. Results.: A total of 52 patients were included. The age group> 60 years (57.70%) was predominant. The female sex represented 69.23% of cases, with a sex ratio of 0.44. Permanent AF was the most found at 63.46%. The EHRA III class (40.38%) was the most represented. The CHA2DS2-VASc score was greater than ≥ 2 in the majority (80.77%) of patients. The HAS-BLED score was less than ≤ 2 in the majority (75.01%) of the cases. The main treatments indicated were: low molecular weight heparin (76.92%), anti-vitamin K (94, 22%), and beta blocker. (90.38%). The most observed complication was: heart failure (53.84%) the lethality was 5.76%. Conclusion. Atrial fibrillation is frequent in our environment, preventive measures and effective management must be put in place to avoid these complications which are potentially serious and disabling.
