Abstract

Introduction. La circoncision est une intervention fréquente chez les garçons. Elle est pratiquée par différentes personnes et est responsable d’un nombre considérable de complications. Le but ce travail est d’analyser les aspects cliniques et thérapeutiques des amputations du gland lors de la circoncision et de sensibiliser sur la pratique traditionnelle de cette intervention. Méthodologie. Il s’agit d’une étude prospective à Koutiala sur une durée de 5 ans. Les amputations du gland suite à la circoncision ont été incluses. Les paramètres étudiés étaient : l’âge, le type de circoncision (l’agent opérateur), avec appréciation du résultat de la qualité de la miction et l’aspect esthétique du pénis. Résultats. Nous avons colligé 21 patients. L’âge moyen était de 5,4 ans. L’amputation du gland était totale dans 18 cas et partielle dans 3 cas. Le traitement repose sur une méatoplastie en cas d’amputation totale et sur une suture de l’albuginé des corps caverneux lésés et du corps spongieux. Conclusion. La circoncision n’est pas une opération tout à fait bénigne. Elle nécessite une formation. L’amputation du gland peut engager le pronostic fonctionnel urinaire et sexuel.
