Urogenital Tuberculosis: A Case Report from Kidal (Mali)

RÉSUMÉ
La tuberculose urogénitale est l’une des localisations fréquentes des formes de tuberculose extra pulmonaire. Nous rapportons un cas de tuberculose urogénitale chez un nomade au nord du Mali dans un contexte d’insécurité. Il s’agit d’un sujet sans notion de contage familiale qui est resté symptomatique pendant 23 ans avec brûlure mictionnelle, pollakiurie, baisse de la libido, dysfonction érectile et éjaculation précoce. Le diagnostic microscopique a été fait après la coloration au Ziehl Nielsen d’échantillons de sperme obtenus en période post coïtale. L’évolution a été favorable sous une quadrithérapie (RHZE) conduite pendant 2 mois laquelle a été substituée une bithérapie par le RH pendant quatre mois. Le caractère sournois de son évolution et sa présentation clinique le plus souvent pauci symptomatique et non spécifique sont à l’origine d’un délai de diagnostic particulièrement long. Devant le moindre signe d’appel urinaire et compte tenu des conséquences potentielles, elle devrait être recherchée systématiquement.

ABSTRACT
Urogenital tuberculosis is a frequent localization of extra pulmonary tuberculosis. We report a case of urogenital tuberculosis in a nomad in northern Mali in a context of insecurity. The subject had no family history of contagion and remained symptomatic for 23 years, with mictional burning, pollakiuria, decreased libido, erectile dysfunction and premature ejaculation.
Microscopic diagnosis was made after Ziehl Nielsen staining of post-coital semen samples. Progression was favourable on 2 months of quadritherapy (RHZE), followed by 4 months of dual therapy with RH. The insidious nature of its evolution and its clinical presentation, which is usually pauci-symptomatic and non-specific, have led to a particularly long delay in diagnosis. At the slightest sign of urinary tuberculosis, and given the potential consequences, it should be systematically investigated.
