Abstract

The diagnosis of gastro duodenal ulcer is rarely evoked in children complaining for abdominal pain. Intestinal parasites are mostly incriminated in the tropical zone. The misdiagnosis of gastro duodenal ulcer in children exposes to complications. The case presented is a 14 year old male adolescent seen at consultation for malaise at school. History revealed recurrent abdominal pain since the age of nine. Several diagnoses were made leading to surgical interventions at 12 and 13 years respectively for umbilical hernia and appendicitis without any relief of symptoms. The last episode of pain associated with melena run for one week. The day before consultation, the child took 500mg of Aspirin for headaches. On physical examination the child looked anemic. Bleeding complicating a gastro duodenal ulcer was suspected and was confirmed by an oeso-gastro-duodenal fibroscopy. Evolution was favourable under Ranitidin and iron tablets. Abdominal pain resolved within 24 hours of treatment
