Abstract

RÉSUMÉ
Objectif. Décrire les aspects  cliniques, thérapeutiques et évolutifs des fentes labio-palatines au Niger. Patients et méthodes. Il s’agit d’une étude rétrospective et descriptive sur les fentes labio-palatines, réalisée du 1er Janvier 2011 au 31 Janvier 2013 dans le service d’odontostomatologie de l’Hôpital National de Niamey et au pavillon  pédiatrique de l’hôpital Cure des enfants de Niamey. Le sexe, l’âge, l’âge maternel, la consanguinité, le rang de naissance, la présentation clinique, la prise en charge et l’évolution étaient les paramètres étudiés. Résultats. Parmi les 2333 patients consultés durant la période de l‘étude, 285 (0,12%) étaient porteurs des fentes labio-palatines dont 160 hommes (56,10%) et 125 femmes (43,85%). Leur âge moyen était de 8,13 ans  (extrêmes 6 ans et 47 ans). La tranche d’âge de 6 à 10 ans (136 patients) était la plus représentée (47,70%) Sur le plan clinique, les formes complètes concernaient 166 patients (58,24%). Les FLP caractérisées par des troubles de succion et de phonation étaient le principal motif de consultation. La chirurgie a été la seule solution thérapeutique curative utilisée (chéiloplastie et palatoplastie). La technique chirurgicale la plus utilisée était celle de Millard (112 patients; 41,20%). Les suites opératoires immédiates et tardives ont été simples. Conclusion. Les FLP affectent plus les garçons que les filles. L‘ âge moyen à la consultation est  de 8,13 ans. Les troubles de succion et de phonation associés à la fente sont les principaux motifs de consultation. Malgré l’âge tardif de la correction, les résultats sont globalement bons.

ABSTRACT
Aim. To describe the clinical presentation, surgical management and outcome of labio-palatal clefts (LBF) in Niger. Patients and methods. This was a cross sectional descriptive retrospective study that was carried out from January 1, 2011 to January 31, 2013. Patinets were recruited in the odonto-stomatology department of the Niamey National Hospital and in the pediatric ward of the Niamey Children's Cure Hospital. Our study parameters were as follows: gender, age, clinical features, management and outcome. Results. Out of 2333 patients who consulted during the study period, 285 (0.12%) with cleft lip and palate were recruited. There were 160 male (56.10%) and 125 female (43.85%). Their mean age was 8.13 years (range: 6 - 47 years). The age group of 6 to 10 years was the most represented with 136 patients (47.70%). There were complete forms 166 patients (58.24%). Sucking and phonation disorders were the main complaints associated with the malformation. Surgery was the sole treatment option, using cheiloplasty and palatoplasty. For 112 patients (41.20%), we used the surgical technique of Millard. The immediate and late aftermath of surgery was simple. Conclusion.  labio-palatal cleftsare more common in boys than in girs. The mean aeg at consultation is 8 years. Sucking and phonation disorders are the main complaints associated with the malformation. Despote the late definitive management, the aftermath of sugery is simple and the overall prognosis is good.
