Greffe de Membrane Amniotique dans la Limbo-Conjonctivite Endémique des Tropiques au Burkina Faso

Abstract

RÉSUMÉ
Le but de notre étude est de présenter l’intérêt de la greffe de membrane amniotique (GMA) dans la Limbo-Conjonctivite Endémique des Tropiques (LCET) au stade IV. Nous rapportons le cas d’une fillette de 11 ans. L’examen ophtalmologique a montré : une acuité visuelle réduite à une perception lumineuse à l’œil droit (OD) et comptait les doigts à l’œil gauche (OG). Les paupières étaient épaisses et les conjonctives hyperémiées. La cornée était envahie sur 360° par un panus carnosus large, ne ménageant qu’un petit espace central clair. Grace à une chaine de solidarité mise en place par une association, la patiente a bénéficié d’une greffe de membrane amniotique. Il y a eu une ré-épithélialisation complète de l’épithélium cornéen avec régression importante des phénomènes inflammatoires. L’acuité visuelle avec la meilleure correction aux deux yeux est passée à 3/10. Elle a recouvré son autonomie grâce à cette intervention. La GMA permet une restauration de sa vision. La membrane amniotique agit donc comme une sorte de pansement qui favorise la repousse des cellules les plus superficielles de la cornée. La procédure est assez simple, susceptible d’être répétée et peut être utilisée efficacement dans plusieurs pathologies cornéennes avec de bons résultats. Sa diffusion dans nos régions est une opportunité pour soulager de nombreux patients.

ABSTRACT
The aim of this study is to discuss the interest of amniotic membrane graft (AMG) in Limbo-Conjunctivitis Endemic of the Tropics (LCET) in stage IV. We report the case of an 11-year-old girl. The ophthalmologic examination showed: a visual acuity reduced to a perception of light at Right Eye and counted the fingers at Left Eye. The eyelids were thick and the conjunctivae hyperemic. The cornea was invaded on 360 ° by a wide carnosus panus, sparing only a small central clear space. Favored by a chain of solidarity set up by an association, the patient benefited from an amniotic membrane graft. There was a complete re-epithelialization of the corneal epithelium with significant regression of inflammatory phenomena. Visual acuity with the best correction in the 2 eyes increased to 3/10. The patient has regained its autonomy from this intervention. The amniotic membrane graft allows restoration of the patient's vision. The amniotic membrane therefore acts as a kind of dressing which favors the regrowth of the most superficial cells of the cornea. The procedure is simple enough to be repeated and can be used effectively in several corneal pathologies with good results. Its diffusion in our regions is an opportunity to relieve many patients.
