Fracture de l’Acromion Associée à une Luxation Acromio-Claviculaire : À Propos d’un Cas

Abstract

RÉSUMÉ
Les auteurs rapportent un cas rare de fracture de l’acromion chez un adulte de sexe masculin ; de suite d’un accident de la voie publique auto-moto pris en charge au service de Traumatologie-Orthopédie et Chirurgie Réparatrice de l’Hôpital National de Niamey. La radiographie de l’épaule de face réalisée a mis en évidence une fracture à trait transversal de l’extrémité distale de la partie postéro-latérale de l’acromion avec un diastasis d’environ 0,5 mm. L’IRM a confirmé les lésions objectivées à la radiographie associées à une subluxation acromio-claviculaire homolatérale. Le traitement chirurgical a consisté à une ostéosynthèse par brochage haubanage sous anesthésie générale, après réduction de la fracture et des séances de rééducation fonctionnelle de l’épaule lui ont été prescrites. Les suites opératoires étaient simples, la consolidation osseuse était obtenue à 4 mois et la récupération fonctionnelle de l’épaule à partir de 6 mois.
ABSTRACT
The authors report a rare case of acromion fracture in an adult male; following a car-motorcycle public road accident treated at the Traumatology-Orthopedics and Reconstructive Surgery department of the National Hospital of Niamey. The AP shoulder X-ray showed a transverse fracture of the distal end of the postero-lateral part of the acromion with a diastasis of approximately 0.5 mm. MRI confirmed the lesions on radiography associated with ipsilateral acromio-clavicular subluxation. Surgical treatment consisted of osteosynthesis by wire bracing under general anesthesia, after reduction of the fracture and functional rehabilitation sessions of the shoulder were prescribed. The postoperative course was simple, bone consolidation was obtained at 4 months and functional recovery of the shoulder from 6 months.
