Abstract

ABSTRACT
Introduction. Worldwide, most patients with chronic kidney disease (CKD) suffer from poor but preventable oral and periodontal health, besides the traditional complications of CKD. Our objective was to evaluate the oral and periodontal health status of patients with chronic kidney disease, and compared to healthy subjects. Materials and methods. We carried out a cross-sectional study from December 2023 to May 2024, in which we enrolled 2 groups of participants which included patients followed up for chronic kidney disease (CKD) at the University Teaching Hospital of Yaounde, Cameroon and a healthy control. Patient was considered healthy if he had no underlined chronic illness. The general oral and periodontal health status were assessed using the Decay Missing Filled Teeth Index (DMFT) and Community Periodontal Index for Treatment Needs (CPITN) respectively. Results. A total of 93 participants were included (50 CKD patients and 43 healthy controls). The number of decayed teeth was lower in the CKD group compared to the healthy controls (p =0.037). CKD patients had fewer filled teeth (p < 0.001) and the overall DMFT score was lower in the CKD population (p= 0.036.) None of the CKD patients had a healthy periodontal status, whereas 11.6% of the controls were healthy (p <0.001). No CKD patient had bleeding gums while 9.3% of controls had (p< 0.001). Calculus was common in both CKD and controls (48.8%). Periodontal Pockets (4-5 mm) were more frequent in CKD patients (p=0.002). Conclusion. CKD patients had better oral but poorer periodontal health compared to healthy controls.
 
RÉSUMÉ
Introduction. Dans le monde entier, la plupart des patients atteints maladie rénale chronique (MRC) souffrent d'une mauvaise santé bucco-dentaire et parodontale, en plus des complications traditionnelles de MRC. Notre objectif était d'évaluer l'état de santé bucco-dentaire et parodontale des patients atteints de la maladie rénale chronique et de le comparer à celui des sujets sains. Matériels et méthodes. Nous avons réalisé une étude transversale de Décembre 2023 à Mai 2024, dans laquelle nous avons enrôlé 2 groupes de participants qui comprenaient des patients suivis pour maladie rénale chronique (MRC) au CHU de Yaoundé, Cameroun et un groupe de témoin sain. Un patient était considéré sain s'il n'avait pas de maladie chronique. L'état général de santé bucco-dentaire et parodontale a été évalué à l'aide de l'indice DMFT (Decay Missing Filled Teeth Index) et de l'indice CPITN (Community Periodontal Index for Treatment Needs) respectivement. Résultats. Au total, 93 participants ont été inclus (50 patients atteints de maladie rénale chroniquent et 43 témoins sains). Le nombre de dents cariées était plus faible dans le groupe des patients atteints de maladie rénale chronique que dans celui des témoins sains (p = 0,037). Les patients atteints de maladie rénale chronique avaient moins de dents obturées (p < 0,001) et le score DMFT global était plus faible dans la population atteinte de maladie rénale chronique (p = 0,036). Aucun des patients atteints de maladie rénale chronique ne présentait un état parodontal sain, alors que 11,6 % des témoins étaient en bonne santé (p < 0,001). Aucun patient atteint de maladie rénale chronique ne présentait de saignement des gencives, alors que 9,3 % des témoins en présentaient (p < 0,001). Le tartre était fréquent tant chez les patients atteints de maladie rénale chronique que chez les témoins (48,8 %). Les poches parodontales (4-5 mm) étaient plus fréquentes chez les patients atteints de maladie rénale chronique (p=0,002). Conclusion. Les patients atteints de la MRC ont une meilleure santé bucco-dentaire mais une moins bonne santé parodontale que les témoins sains.
