Abstract
RÉSUMÉ
La régurgitation mitrale sévère secondaire à la rupture du muscle papillaire est une complication rare mais dramatique de l'infarctus du myocarde, en raison de la mortalité associée. Elle nécessite un diagnostic précoce et un traitement urgent. Pour des raisons anatomiques, la rupture du muscle papillaire postéro-médian est plus fréquente et est le plus souvent associée à un prolapsus de la valve mitrale postérieure. L'échodoppler cardiaque transthoracique reste la technique d'imagerie de première intention pour le diagnostic, mais l'échocardiographie transoesophagienne est l'examen le plus précis. Nous rapportons un cas d'insuffisance mitrale par prolapsus de la valve mitrale antérieure secondaire à une rupture du muscle papillaire postéro-médian chez un homme de 42 ans admis pour un syndrome coronarien aigu secondaire à une occlusion de l'artère coronaire droite.
ABSTRACT
Severe mitral regurgitation secondary to papillary muscle rupture is a rare but dramatic complication of myocardial infarction, due to the associated mortality. It requires early diagnosis and urgent treatment. For anatomical reasons, rupture of the posteromedial papillary muscle is more common and is most often associated with posterior mitral valve prolapse. Transthoracic cardiac duplex remains the first-line imaging technique for diagnosis, but transesophageal echocardiography is the most precise examination. We report a case of mitral regurgitation due to prolapse of the anterior mitral valve secondary to rupture of the posteromedial papillary muscle in a 42-year-old man admitted for acute coronary syndrome secondary to occlusion of the right coronary artery.
