Abstract

RÉSUMÉ
Introduction. La fibrillation atriale est le trouble du rythme cardiaque soutenu le plus fréquent. Les objectifs de notre travail étaient de déterminer la prévalence hospitalière et de décrire les facteurs étiologiques, l'évolution et le traitement de la fibrillation atriale dans le service de médecine générale de l’hôpital de Tombouctou au Mali. Méthodes. Il s'agit d'une étude transversale descriptive réalisée sur un an et portant sur l’analyse de 32 dossiers de patients hospitalisés pour une fibrillation atriale ou une pathologie compliquée de fibrillation atriale. Résultats. La fréquence de la fibrillation atriale était de 4,8% avec une prédominance masculine (sex-ratio : 1,28). L’âge moyen était de 59 ans avec des extrêmes de 25 et 87 ans. Les deux étiologies principales étaient l’hypertension artérielle (46,8%) et les valvulopathies (12,5%). Les complications thromboemboliques étaient plus fréquentes en cas de fibrillation atriale non valvulaire, et chez les patients âgés de plus 60 ans avec une dilatation atriale gauche. Une anticoagulation par anti-vitamine K a été instaurée dans 56,3% des cas. Les bêtabloquants (78,1%) et la digoxine (9,4%) étaient les antiarythmiques les plus prescrits. Conclusion. La fibrillation atriale est fréquente en milieu hospitalier africain et concerne des sujets âgés en dehors des atteintes valvulaires. L’hypertension artérielle et les valvulopathies rhumatismales sont les principaux facteurs étiologiques. Le traitement reste essentiellement médicamenteux par contrôle de la fréquence cardiaque.

ABSTRACT
Introduction. Atrial fibrillation is the most common sustained heart rhythm disorder. The objectives of our work were to determine the hospital prevalence and to describe the etiologicies, the evolution and the treatment of atrial fibrillation in the general medicine department of the hospital of Tombouctou in Mali. Methods. This was a descriptive cross-sectional study carried out over one year. It concerned the records of 32 patients hospitalized for atrial fibrillation or a pathology complicated by atrial fibrillation. Results. The hospital frequency of atrial fibrillation was 4.8% with a predominance of men (sex ratio: 1.28). The mean age of patients was 59 with extremes of 25 and 87. The main etiologies were arterial hypertension (46.8%) and valvular disease (12.5%). of cases. Thromboembolic complications were more common in nonvalvular atrial fibrillation and patients older than 60 years with left atrial dilation. Anticoagulation with anti-vitamin K was initiated in 56.3% of patients. Beta-blockers (78.1%) and digoxin (9.4%) were the most common antiarrhythmics that were administered. Conclusion. Atrial fibrillation is frequent in African hospitals. Il concerns mainly elderly subjects and patients with valvular damage. High blood pressure and rheumatic valve disease are the main causative factors. The treatment is mainly medical aiming at controlling the heart rate.
