RÉSUMÉ
Introduction. L’hématome extradural (HED) du nourrisson est en croissance dans nos pays. Il s’agit d’une urgence absolue, dont le diagnostic repose sur le scanner cérébral et la thérapeutique peut être conservatrice ou chirurgicale. L’objectif de cette étude est de décrire le profil épidémiologique de nos patients et d’évaluer la prise en charge diagnostique et thérapeutique ainsi que le devenir de nos patients. Patients et méthodes. Il s’agit d’une étude transversale, rétrospective portant sur 23 nourrissons hospitalisés pour hématome extra dural, pris en charge au service de Neurochirurgie du Centre Hospitalier National Universitaire de FANN (CHNU) à Dakar, du 1er janvier 2011 au 31 décembre 2018. Les variables d’étude étaient l'âge, le tableau clinique initial, les résultats du scanner ainsi que le traitement et l’évolution des patients. Résultats. L’hématome extra dural représentait 7.4% des traumatismes cranio-encéphaliques du nourrisson. L’âge moyen était de 13.5 mois. Le sex ratio G/F était de 4.7. Les accidents domestiques représentaient les circonstances de survenue les plus fréquentes et les mécanismes étaient dominés par une chute du lit. Le score de Blantyre moyen à la présentation était de 3,9. Le délai de consultation moyen était de deux jours. Dans 78.3% des cas (n=18), on observait une seule lésion au scanner cérébral. Six enfants (26%) ont bénéficié d’un traitement chirurgical. L’évolution globale après 12 mois a été favorable dans 91.3% de cas. Conclusion. L’état clinique initial, le délai de la prise en charge peuvent influencer le pronostic des nourrissons présentant un HED. Le pronostic est en règle excellent si la prise en charge est bien codifiée.
ABSTRACT
Introduction. Infant epidural hematoma (EH) is an absolute emergency. Its incidence is growing in our country. The diagnosis is based on head CT. The treatment of ED is either conservative or surgical.  The objective of our study was to describe the clinical presentation, the diagnostic features and the treatment in a group of Senegalese infants.  Methods: This was a cross-sectional, retrospective study of 23 infants admitted for ED and managed at the Neurosurgery Department of CHNU in Fann. Our study variables were age, initial clinical presentation, CT scan results, treatment and outcome.  Results. ED accounted for 7.4% of all head injuries in infants. The mean age was 13.5 months. The sex ratio was 4.7. Domestic accidents were the most frequent circumstances of occurrence and the mechanism was mainly falling from bed. The mean Blantyre score at presentation was 3.9. In 78.3% of cases (n=18), there was a single lesion on CT. Six patients (26%) were operated. The overall evolution after 12 months was good in 91.3% of cases.  Conclusion. Initial clinical status and time delay before treatment greatly influence the prognosis of infant ED. The prognosis is generally good.
