Abstract

RÉSUMÉ
La sténose de l’artère carotidienne est définie par une réduction de sa lumière par une plaque d’athérosclérose, artérite, ou une dysplasie fibromusculaire. Nous décrivons un cas de syndrome d’ischémie oculaire (SIO), qui a révélé la maladie cardio- vasculaire. Il s’agit d’un patient de 65 ans, qui a consulté pour une baisse de l’acuité visuelle bilatérale et une douleur intense à l’œil gauche évoluant depuis moins de 24 heures. L’examen ophtalmologique a retrouvé des signes d’ischémie oculaire. L’échographie doppler des vaisseaux du coup réalisée, après des syncopes brèves répétées, a fait le diagnostic. Dans notre cas, l’hypothèse émise est l’obstruction de l’artère ophtalmique par un caillot détaché de la plaque d’athérome. Cette obstruction est à l’origine de l’hypo perfusion des globes oculaires. L’ischémie rétinienne ayant entrainée la formation de néo vaisseaux et par conséquent le développement de glaucome.
ABSTRACT
Carotid artery stenosis by a reduction of of lumen by a plaque of atherosclerosis arteristis or dysplasia fibromuscular. We describe a case of ocular ischemia syndrom (OIS) wich revealed vascular disease. This was a 65 year old patient who consulted for a decrease in bilateral visual acuity and severe pain in the left eye that has has been progressing for less than 24 hours. OIS signs were present at the ophthalmologic examination. Carotid doppler ultra sound confirmed the diagnosis. In this case , the hypothesis was obstruction of the ophtamic artery by a clot detached from the artheroma plaque. Retinal ischemia lead to the formation of the new vessels and development of glocoma.
