RÉSUMÉ
Introduction. L’enclouage centromédullaire par clou de Küntscher permet un montage stable et solide. Le but de l’étude était d’évaluer les résultats de cette technique en urgence dans le type III de Gustilo. Patients et Méthodes. Il s’est agi d’une étude prospective mono centrique de janvier 2017 à décembre 2018. Étaient inclus les patients traités en urgence pour une fracture ouverte type III de Gustilo par réduction ouverte et enclouage centromédullaire au clou de Küntscher. Une antibiothérapie parentérale avec Amoxicilline/Acide Clavulanique et métronidazole était systématique. Il était réalisé un lambeau de couverture dans le même te mps opératoire quand les conditions locales étaient favorables. L’évaluation des résultats anatomiques et fonctionnels a été faite selon les critères modifiés de Ketenjian. Résultats. La série était composé de 30 hommes et 11 femmes. Selon Gustilo/Anderson il y avait : 9 (20,5%) fracture type IIIA et 35 cas (79,5%) type IIIB. Le délai moyen opératoire était de 34,09 ± [6 -72] heures. Dans 70,5% des cas le montage était stable d’emblée et dans 29,5% des cas il a été complété par une immobilisation plâtrée. Le lambeau local était immédiat dans 16 et différé dans 38 cas. Les complications étaient: une infection de la plaie (25%) et une pseudarthrose (6,8%). Le délai moyen de consolidation a été de 5,83 mois. Les résultats fonctionnels et anatomiques ont été au moins bons dans 65,9 % des cas. Conclusion. L’enclouage centromédullaire en urgence dans les fractures ouvertes de jambe permet d’avoir de bons résultats avec l’avantage d’être moins onéreux dans un contexte de sous équipement.
ABSTRACT
Purpose. The aim of the study was to evaluate the results of kuntscher intramedullary nailing in emergency in open tibia shaft fracture type III. Patients and Methods. This was a series of prospective single-center cases carried from January 2017 to December 2018. It included patients with open tibia shaft fracture type III, managed by Kuntscher intramedullary nail. Parenteral antibiotic therapy with Amoxicillin / Clavulanic acid and metronidazole were systematic. A cover flap was performed during the same procedure when local conditions were favorable. Anatomical and functional results were assessed by the Ketenjian criteria. Results. The series consisted of 30 men and 11 women. According to the Gustilo/Anderson classification opened fracture were: IIIA in 9 cases (20.5%) and type IIIB in 35 cases (79.5%). The mean delay of management was 34.09 ± [6 – 72] h. The intramedullary nailing was in 70.5% stable and in 29.5% of cases unstable and it was completed by a cast immobilization. Immediate flap covering was performed in 16 cases and flap delay in 38 cases. Traumatic wound infection occurred in 25% of patients. The length of hospital stay was on average 24.34 days. The mean delay to union was 5.83 months. Non-union occurred in 6.8% of cases. The functional and anatomical results were at least good in 65.9% of cases. Conclusion. Intramedullary nailing in open tibia fractures provides good results with the advantage of being less expensive in low-income setting.
