Abstract

Introduction : L’inégale répartition dans le monde  des moyens endoscopiques et radiologiques pour le diagnostique des ictères obstructifs a conduit à une absence de consensus dans la prise en charge des patients.

Objectifs : Les objectifs de cette étude étaient d’analyser les méthodes de diagnostic et de traitement des patients présentant un ictère obstructif à Yaoundé, et de proposer des changements d’attitudes  dans la prise en charge de ces patients.

Méthodologie : Il s’agissait d’une étude rétrospective descriptive transversale portant sur l’analyse des dossiers des patients traités dans le Service de Chirurgie du CHU de Yaoundé pendant une période de dix ans allant de 1999 à 2009. Les variables d’étude étaient les signes et les symptômes, les moyens de diagnostic paraclinique, le diagnostic peropératoire et les méthodes de traitement chirurgical.

Résultats : Trente dossiers de patients avec ictère obstructif ont été inclus dans l’étude. Il y avait 18 hommes et 12 femmes, âgés de 45 à 73  ans avec une moyenne de 55 ans. Une patiente de 77 ans avait été exclue pour insuffisance cardiaque. La fibroscopie oesogastroduodénale réalisée chez six patients n’avait eu aucun intérêt diagnostique. L’échographie réalisée chez les 30 patients avait montré une dilatation des voies biliaires dans 28 cas (93,3%). La tomodensitométrie avait été faite chez 21 patients (70%). Elle avait évoqué le diagnostic de cancer de la tête du pancréas dans 15 cas (50%). Tous les patients avaient été traités chirurgicalement par une laparotomie, avec un diagnostic final de 16 cas de cancer du pancréas (53,3%). Le traitement chirurgical avait consisté en une dérivation biliodigestive dans 27 cas (90%) et en une duodénopancréatectomie céphalique avec une évolution fatale. Vingt-deux biopsies ont permis le diagnostic de 4 cas de pancréatite chronique non suspectés en préopératoire (13,3%). Il y eu deux décès, soit un taux de mortalité de 6,6%.

Conclusion : L’étude  confirme l’insuffisance de l’échographie et de la tomodensitométrie comme  seules méthodes de diagnostic des ictères obstructifs. Elle recommande l’introduction des méthodes mini-invasives  endoscopiques et laparoscopiques par  une équipe multidisciplinaire dans la prise en charge des ictères obstructifs à Yaoundé.

 

Introduction: The unequal distribution of recent endoscopic and imaging methods of diagnosis of obstructive jaundice in the world has lead to an absence of consensus in the management of patients presenting that condition.

Objectives: The objective of this study were to analyse the methods of diagnosis and treatment of patients with obstructive in Yaoundé in order to propose changes in our practice to reduce the morbidity and mortality rates.

Methodology: The study was a retrospective descriptive cross-sectional analysis of patients’ files following surgical treatment of jaundiced patients in the Surgical Unit of the University Teaching Hospital of Yaoundé during a ten-year period extending from 1999 to 2009. The variables analysed were clinical signs and symptoms, diagnostic preoperative investigations, intra-operative findings, and methods of surgical treatment.

Results: The files of thirty patients were included in the study. There were 18 males and 12 females aged 45 to 73 years with an average of 55 years. A 77 year-old woman was excluded for cardiac failure. All patients presented clinical jaundice. Gastroduodenal fibroscopy had been done on 6 patients with no contribution in the diagnosis. Echography had been performed on all patients with the diagnosis of dilatation of bile ducts in 28 cases (93.3%) and CT-scan on 21 patients (70%) with the diagnosis of cancer of pancreas in 15 cases (50%). All patients were treated by open abdominal surgery leading to the diagnosis of 16 cancers of the pancreas (53.3%). In five patients (16.6%) there was no obstacle on manual exploration of the common bile duct. Biliary by-pass had been done on 27 patients (90%). There was one pancreaticoduodenectomy with a fatal evolution. Twenty two biopsies had revealed the diagnosis of four cases of unsuspected chronic pancreatitis (13.3%).Two patients had died, giving a mortality rate of 6.6%.

Conclusion: We conclude that use of echography and CT-scan as the only preoperative diagnostic investigations on patients with obstructive jaundice is inadequate. We recommend the use of endoscopy and laparoscopy in a multidisciplinary team to improve the quality of the diagnosis of jaundiced patients, and mini-invasive surgery in general practice.
