RÉSUMÉ
Introduction. L’objectif du travail est de décrire les aspects radiologiques des malformations congénitales de l’appareil urinaire chez l’enfant de 0 à 15 ans à Bamako (Mali). Patients et méthodes. Il s’agit d’une étude prospective, descriptive réalisée dans le service de Radiologie et d’Imagerie médicale du CHU du Gabriel Touré du 01 février 2016 au 31 janvier 2017. Nous avons inclus tous les enfants de 0 à 15 ans chez qui le diagnostic d’uropathie malformative était posé. Les études morphologiques et fonctionnelles de l’appareil urinaire ont été effectués avec un appareil de radiographie de marque Siemens, un échographe de marque Philips muni de trois sondes multifréquences et un appareil de tomodensitométrie de marque Siemens 16 slices. Résultats. Nous avons retenu 33 cas d’uropathies malformatives soit une fréquence de 2,4%. La tranche d’âge la plus touchée était de 0-5 ans avec un sex-ratio 2,3. Les troubles mictionnels étaient le principal motif de consultation avec 42,4%. L’échographie a été l’examen le plus réalisé avec 100% des cas de notre série, suivie de la TDM soit 39,4% des cas. Les agénésies rénales et le syndrome de jonction pyélo-urétérale prédominaient au niveau du haut appareil urinaire avec 12,1% respectivement. La valve de l’urètre postérieur était l’anomalie la plus représentée au niveau du bas appareil urinaire avec 21,2%. Une fonction rénale altérée était retrouvée dans 15,2%. Conclusion. Les uropathies malformatives sont de plus en plus fréquentes dans la pratique courante à Bamako. L’imagerie médicale nous a permis d’apprécier la morphologie et le retentissement de la malformation sur l’appareil urinaire.
ABSTRACT
Introduction. The objective of the study was to describe radiological presentation of congenital malformations of the urinary tract of children aged 0 to 15 years in Bamako (Mali). Patients and methods. This was a cross sectional prospective and descriptive study conducted in the Radiology and Medical Imaging Department of the Gabriel Touré University Hospital from 01 February 2016 to 31 January 2017. All children aged 0 to 15 years with a diagnosis of malformative uropathy were included. Morphological and functional studies of the urinary tract were performed with a Siemens X-ray machine, a Philips ultrasound machine with three multi-frequency probes and a Siemens 16 Slices CT equipment. Results. We studied 33 cases of malformative uropathy, a frequency of 2.4%. The most affected age group was 0-5 years and the sex ratio was 2.3. Mictin difficilties were the main reason for consultation in 42.4% of cases. Ultrasound was performed in 100% of the cases, followed by CT scan in 39.4% of cases. Renal agenesis (12.1%) and pyeloureteral junction syndrome (12.1%) were the most common anomalies of the upper urinary tract. Posterior urethral valve was the most common anomaly in the lower urinary tract (21.2%). Impaired renal function was found in 15.2%. Conclusion. Malformative uropathies are becoming more and more frequent in the current practice in Bamako. Medical imaging allowed us to appreciate the morphology and the impact of the malformation on the urinary system.
